The problem
A pharmacist checking a tablet behind the counter of a fully stocked pharmacy.

Carriers, third-party administrators, and pharmacy benefit managers profited $603B in a single year — money taken out of premiums before anyone saw a doctor.

The model you’re paying for

It isn’t expensive because healthcare is expensive.

It’s expensive because four companies get paid before your doctor does. Every one of them adds margin, paperwork, and a reason to say no.

10–18%

Premiums climb every year

Costs go up annually while coverage barely improves. Renewal season becomes a negotiation you always lose.

$603B

Middlemen take a cut first

Carriers, third-party administrators, and pharmacy benefit managers profited $603B in a single year. That money comes out of premiums before anyone sees a doctor.

8–10×

Billed far above cash price

Traditional insurers routinely charge many times what the same care costs paid directly — then apply a deductible on top of it.

$0

Coverage you can't afford to use

A deductible high enough to deter care isn't coverage. People skip the appointment, the condition worsens, and the eventual claim is larger.

Where the money goes instead

Four things we removed.

None of this is a discount program layered on top of insurance. It’s a different supply chain — and each link we took out is a cost that never reaches you.

  1. 01

    No carrier in the middle

Reef is the end provider, not a reseller. There is no insurance carrier, third-party administrator, or pharmacy benefit manager taking a cut of your premium before care is delivered.

  2. 02

    Direct-to-provider pricing

Rates are negotiated directly with doctors, labs, imaging centers, and pharmacies. You see the price before the visit, not in a bill six weeks later.

  3. 03

A care team that does the work

Everyday 1to1® Care Navigation schedules the appointment, tells the office what you owe, explains how to bill the network, and negotiates the rate. You do not chase anyone.

  4. 04

    Pharmacy without the PBM

Reef works directly with pharmacies. Generics and immunizations come at no cost on every medical plan, and brand-name drugs are discounted through direct partnerships.

Everyday 1to1® Care Navigation

A real person does the hard part.

Most plans hand you a card and a phone tree. Reef hands you a care team that makes the calls, knows the prices, and tells the provider’s office how to bill before you walk in.

  • You describe what's wrong in the app.

A provider calls you at a time you picked.

  • You say which doctor you'd rather see.

They add that doctor to the network with 48 hours' notice.

  • You need labs, imaging, or a specialist.

They find the lowest price and book it near you.

  • You get a bill you weren't expecting.

They call the provider and get it billed correctly.

  • You need surgery.

They negotiate the rate and tell you your cost beforehand.

How to use your plan

Straight answers

The questions everyone asks first.

Reef isn't a variation on the plan you already have, so the questions are usually about how it can possibly work. Here's the honest version.

How is Reef Health different from traditional health insurance?

Reef Health is not traditional insurance. It is a direct-pay health benefits model built for small businesses, solopreneurs, families, and individuals. Traditional insurers add layers of intermediaries — carriers, third-party administrators, and pharmacy benefit managers — that each take a cut before care is delivered, which is how billed amounts end up many times higher than the cash price for the same service. Reef Health removes those intermediaries, works directly with providers and pharmacies, publishes what care costs before the visit, and assigns every member a care team that schedules appointments and negotiates rates on their behalf.

It sounds too good to be true. How can Reef Health cost this much less?

The savings come from removing cost, not from reducing care. Reef Health is the end provider rather than a reseller, so there is no carrier markup on the plan. Rates are negotiated directly with doctors, labs, imaging centers, and pharmacies, which eliminates the pharmacy benefit manager margin on prescriptions and the administrative overhead of claims routed through multiple parties. Pricing is published upfront, so there is no surprise-billing cycle to fund. Reef Health plans are powered by Redirect Health, which has delivered more than $168M in savings to members.

Does Reef Health cover all 50 states?

Yes. Reef Health provides nationwide coverage, and pricing is the same in every state. Members in all 50 states can access 24/7 virtual care and telehealth, direct provider and hospital care, and concierge care navigation. In-person care runs through the PHCS Practitioner Only Network operated by MultiPlan, which includes more than 800,000 doctors, hospitals, and healthcare professionals.

Do I have to switch doctors to use Reef Health?

No. Reef Health members keep their preferred doctors. Members can use the direct provider network, visit an out-of-network provider with cost guidance from the care team, or use virtual care. If a preferred doctor is not currently in the network, members can request to add them by notifying the care team at least 48 hours before the visit — the team negotiates a Usual, Customary, and Reasonable agreement, and Reef Health considers itself an open network because any doctor can be onboarded this way.

What healthcare services are included in Reef Health plans?

Depending on the tier, Reef Health plans include 24/7 virtual primary care, in-office primary and urgent care, pediatric care and well-child visits, annual physicals, labs and imaging coordination, prescription savings with $0 generic medications and immunizations, mental health tele-counseling, chiropractic care, specialist coordination, dental and vision discounts, and hospital coverage for serious medical events on the Premium + tier.

Is a Reef Health plan a Minimum Essential Coverage plan under the ACA?

The Premium and Premium + tiers meet Minimum Essential Coverage requirements under Part A of the Affordable Care Act. For employers, ACA affordability under Part B is calculated as a percentage of an employee's household income, and Reef Health's employer-sponsored plans are structured so Applicable Large Employers with 50 or more employees can meet minimum contribution requirements at a lower cost than traditional group coverage. Reef Health can confirm current-year affordability thresholds for a specific plan design.
